Appropriates $7,500,000 for fiscal year 2017-2018 and $7,500,000 for fiscal year 2018-2019 to the Department of Health and Human Services, Office of Rural Health for the Community Health Centers Grant Program.

Establishes eligibility requirements for grant recipients including free clinics, federally qualified health centers, rural health centers, local health departments, and nonprofit organizations serving populations that are at least 80% uninsured or underinsured/Medicaid/Medicare recipients.

Authorizes grants to increase access to primary care and preventative health services, establish new services in underserved counties, create dental/pharmacy/behavioral health services, and increase primary care capacity.

Limits individual grant awards to $150,000 per fiscal year and restricts use of funds from enhancing personnel compensation, supplanting existing federal funds, or financing existing debt.

Requires grant recipients to report measurable quality health outcomes, patient numbers, types of services provided, and other performance metrics to the Office of Rural Health; allows up to $300,000 annually for program administration.
