Creates the Healthy Food Small Retailer Fund within the Department of Agriculture and Consumer Services to provide funding and assistance to small food retailers in food desert zones.

Defines eligible small food retailers as businesses with no more than 5,000 heated square feet, including corner stores, convenience stores, and bodegas, located in areas meeting specific poverty and food access criteria.

Authorizes use of fund money for purchasing and installing refrigeration and display equipment (up to $5,000 per retailer), offsetting initial participation expenses (up to $100 per retailer), and providing technical assistance on food safety and nutrition (up to $1,500 per retailer).

Requires small food retailers receiving assistance to accept or agree to accept Supplemental Nutrition Assistance Program (SNAP) and Women, Infants, and Children (WIC) benefits, and mandates they enter performance agreements with county or regional public health departments.

Requires the Department of Agriculture to publish annual reports on fund usage, geographic distribution of disbursements, and health outcomes by April 30 of each year to specified legislative committees.
