Appropriates $300,000 for fiscal year 2017-2018 and $200,000 for fiscal year 2018-2019 to the Department of Health and Human Services, Division of Aging and Adult Services.

Allocates funds to the Center of Outreach in Alzheimer's, Aging, and Community Health at North Carolina Agricultural and Technical State University to develop evidence-based workshops, support groups, and conferences on Alzheimer's disease and related dementias in 20 counties with the highest number of low-income families.

Requires development of culturally relevant, bilingual informational materials and a comprehensive caregiver toolkit covering financial resources, legal planning, elder care, and local resources, to be made available free on state websites.

Establishes a "Connect the Family" project to increase awareness and inform communities about available resources, including conducting a statewide caregiver's education conference in the 20 identified rural counties, either remotely or in person.

Becomes effective July 1, 2017.
