Extends license revocation penalty from one year to five years for applicants who previously owned or operated a facility with a revoked license, and adds new five-year penalties for applicants involved in summarily suspended facilities or those with outstanding unpaid fees and fines.

Establishes informal dispute resolution process allowing adult care homes to challenge inspection findings from both the Division of Health Service Regulation and county departments of social services before enforcement actions are imposed.

Increases minimum training requirement for personal care aides from 75 hours to 80 hours, with exemptions available for licensed health care professionals or those on the Nurse Aide Registry.

Eliminates the 12-24 month penalty period during which Type A and Type B violations affect facility star ratings, changing to ratings based only on current inspections and quality improvement participation.

Exempts acquisitions of unlicensed adult care homes that were licensed within the preceding 12 months from certificate of need review requirements.
