Establishes the "Carolina Cares" program to provide health coverage to North Carolina residents ages 19-64 with household income up to 133% of federal poverty level who are ineligible for Medicaid and not enrolled in Medicare.

Requires participants to pay monthly premiums set at 2% of household income, with exemptions for those below 50% of federal poverty guidelines, those with medical or financial hardship, tribal members, and veterans in transition.

Mandates program participants be employed or engaged in employment-promoting activities, with exemptions for individuals caring for dependents, those in substance abuse treatment, and those determined to be medically frail.

Provides a benefit package similar to North Carolina's 2017 Essential Health Benefits Benchmark Plan with emphasis on preventative care, managed through Prepaid Health Plans under capitated contracts.

Limits funding sources to federal funds, participant contributions, and state funds from healthcare-related assessments (such as hospital assessments), with implementation contingent on sustainable funding availability; requires DHHS to submit a design proposal and federal waiver strategy by January 1, 2018.
