Requires applicants for pharmacist licensure to provide a criminal record report obtained from Board-designated reporting services.

Applicants must submit all application materials including name, age, pharmacy study history, and prescription compounding experience as specified on the Board's form.

Establishes four qualifications for licensure: holding an undergraduate degree from an approved pharmacy school, completing up to one year of supervised experience, passing the required Board examination, and demonstrating physical and mental competency.

Requires the Board to keep all criminal record information confidential and not a public record under Chapter 132 of the General Statutes, in accordance with state and federal guidelines.

Applicants are responsible for paying the designated reporting service for the cost of obtaining their criminal record reports.

Effective January 1, 2018, for applications submitted on or after that date.
