Appropriates $300,000 for fiscal year 2017-2018 and $200,000 for fiscal year 2018-2019 to the Department of Health and Human Services, Division of Aging and Adult Services.

Allocates funds to the Center of Outreach in Alzheimer's, Aging, and Community Health at North Carolina Agricultural and Technical State University to develop evidence-based workshops, support groups, and conferences targeting 20 counties with the highest number of low-income families, with focus on underserved, underrepresented, and rural populations.

Requires development of culturally relevant, bilingual informational materials about Alzheimer's disease and related dementias to be distributed at events and made available free online.

Directs creation of a comprehensive caregiver toolkit addressing financial resources, legal planning, elder care, and local resources to be made available free online.

Establishes a "Connect the Family" project to conduct statewide caregiver education conferences in the 20 identified rural counties, either remotely or in person, to share research information and introduce the caregiver toolkit.
