Directs the Department of Health and Human Services, Division of Medical Assistance to provide Medicaid and NC Health Choice coverage for evidence-based home visits for pregnant women and families with young children by July 1, 2018, either statewide or through a pilot program.

Requires the Department to develop a plan detailing the design, scope, service definition, visit schedule, and reimbursement rates for home visit coverage, consistent with the Nurse-Family Partnership model.

Mandates submission of a comprehensive report to the Joint Legislative Oversight Committee on Medicaid and NC Health Choice and the Fiscal Research Division by November 1, 2017, including fiscal impact analysis, implementation timeline, and details on leveraging private funding.

Prohibits the Department from making changes to existing Medicaid or NC Health Choice eligibility requirements in implementing the plan.

Authorizes the General Assembly to appropriate funds for implementation during the 2018-2019 fiscal year if the plan cannot be implemented under existing Department authority.
