Requires registered voters presenting to vote in person to produce photo identification, including driver's licenses, U.S. passports, North Carolina voter photo ID cards, tribal enrollment cards, student IDs, and employee IDs issued by state or local government entities.

Establishes free North Carolina voter photo identification cards issued by county boards of elections valid for 10 years, available on request no later than May 1, 2019.

Allows voters without acceptable photo identification to cast provisional ballots by completing affidavits citing religious objections, reasonable impediments (such as lack of transportation or disability), or victim status from natural disasters occurring within 100 days before the election.

Requires provisional ballots cast due to lack of photo identification to be counted only if the voter presents acceptable identification to the county board of elections by end of business the day before the canvass.

Appropriates $2.25 million to the State Board of Elections and Ethics Enforcement for implementation, including $750,000 for creating temporary positions and $1.5 million transferred to the Highway Fund to offset revenue losses from issuing free identification.
