Allows retail sale of raw milk for human consumption in North Carolina, subject to permits and quality standards.

Defines "small herd operation" as a dairy farm with no more than 10 lactating animals (cows, goats, or sheep combined), which qualify for a simplified Small Herd Raw Milk permit exempt from certain sanitation and construction requirements.

Establishes mandatory quality standards for raw milk including cooling to 40°F within two hours, bacterial limits of 10,000 per milliliter, coliform limits of 10 per milliliter, and disease testing requirements for brucellosis and tuberculosis.

Requires raw milk sold off-premises to be labeled with a 9-day sell-by date, safety warning in at least one-half inch letters, refrigeration instructions, species identification, and producer information; prohibits sales through restaurants.

Permits raw milk dispensing for personal consumption to co-owners of animals without a permit, but maintains existing Department of Agriculture authority over labeling, testing, and other regulations.
