Establishes a two-year pilot program in Cumberland County (ending June 30, 2021) to provide health care services to veterans through the Department of Health and Human Services and Department of Military and Veterans Affairs, in coordination with Community Care of North Carolina and Maxim Healthcare Services.

Includes a health care initiative providing veterans increased access to health care resources through community health worker care coordination efforts.

Includes a workforce initiative to recruit and train unemployed and underemployed veterans as community health workers for the health care initiative.

Appropriates $400,000 in nonrecurring funds for fiscal year 2019-2020 and $350,000 in nonrecurring funds for fiscal year 2020-2021 to support program development and implementation.

Requires the Department of Health and Human Services to submit a comprehensive evaluation to the Joint Legislative Oversight Committee on Health and Human Services by February 1, 2021, including program outcomes and fund expenditures.
