Establishes the North Carolina Medical Cannabis Act to allow qualified patients with debilitating medical conditions to possess and use cannabis for medical purposes under a regulated system.

Creates a registry identification card system administered by the Department of Health and Human Services for qualified patients and designated caregivers, with cards valid for two years.

Establishes a regulated medical cannabis supply system through the Department of Agriculture and Consumer Services with licensed medical cannabis centers, producers of medical cannabis, and producers of cannabis-infused products; all licensees must pay $5,000 initial fees and 10-15% monthly gross revenue fees.

Protects qualified patients, designated caregivers, physicians, and licensed producers from arrest, prosecution, and penalties when acting in compliance with the Act; provides affirmative defenses for medical cannabis possession and use.

Limits qualified patient possession to an "adequate supply" (not exceeding 24 ounces for smoking/vaping) and prohibits cannabis use while operating vehicles, on school grounds, or in public places; temporary certificates available during implementation period.
