Requires any person requesting or delivering an absentee ballot application and ballot form for a voter in a nursing home, assisted living residence, or residential facility to first consult that voter's near relative or verifiable legal guardian

Absentee ballot request forms must include the voter's name and address, the near relative or legal guardian's name and address if they are making the request, the mailing address for ballots if different from residence, identification per State Board rules, voter's date of birth, and signature of voter or their representative

Applies to all written requests for absentee applications and ballots submitted on or after the date the act becomes law
