Extends the mandatory connection deadline for most Medicaid and State-funded health care providers to June 1, 2020, from the original June 1, 2019 deadline.

Delays connection deadlines for ambulatory surgical centers, dentists, psychiatrists, pharmacies, and state health care facilities to June 1, 2021.

Allows the Department of Information Technology to grant limited time extensions for providers demonstrating good-faith efforts, with a maximum extension deadline of June 1, 2022.

Exempts providers from penalties and claim denials if they fail to meet the June 1, 2019 deadline but successfully connect by June 1, 2020.

Authorizes the Secretary of Health and Human Services to grant hardship exemptions to classes of providers for whom electronic health record implementation would constitute undue hardship, with a final compliance deadline of December 31, 2022.
