Amends G.S. 7B-2411 to require written adjudication orders in juvenile delinquency cases to include four specific findings of fact and conclusions of law: the date of offense, misdemeanor or felony classification (if delinquent), date of adjudication, and facts relied upon in the delinquency determination.

Adds a new requirement that courts must document "the facts relied upon in adjudicating the juvenile delinquent" in all juvenile delinquency adjudication orders.

Maintains existing provisions that if allegations are not proved, the court shall dismiss the petition with prejudice and release the juvenile from custody.

Effective immediately upon enactment and applies to all adjudication orders entered on or after the effective date.
