Establishes a State Office of Child Fatality Prevention within the Department of Health and Human Services, Division of Public Health, to coordinate and oversee the statewide Child Fatality Prevention System as the lead agency.

Appropriates $150,000 for fiscal year 2019-2020 and $300,000 for fiscal year 2020-2021 to establish and operate the State Office.

Requires the Department of Health and Human Services to develop a transition plan by March 4, 2020, to consolidate child fatality review teams, reduce redundancy, and create a centralized data and information system using the National Child Death Review Case Reporting System.

Formalizes the North Carolina Child Fatality Task Force structure with three committees (Perinatal Health, Unintentional Death Prevention, and Intentional Death Prevention), specifies leadership roles, and clarifies reporting duties to the Governor and General Assembly.

Modifies statutes governing child death review teams, the State Team, Local Teams, and Community Child Protection Teams to align with the new centralized coordination structure and expand the Task Force membership from 35 to 36 members.
