Establishes the Group Home Stabilization and Transition Fund with $26,000,000 in nonrecurring funds for fiscal year 2019-2020 and $26,000,000 for fiscal year 2020-2021 to support community-based group homes licensed under Chapter 122C.

Allocates appropriated funds to incentivize local management entities/managed care organizations (LME/MCOs) to develop Medicaid-funded "in-lieu-of" services and transition group home residents to sustainable funding sources while increasing direct support staff wages.

Requires DHHS to implement by January 1, 2020 an actuarially sound rate model for new Medicaid services specific to group homes serving residents with intellectual or developmental disabilities and mental health needs.

Directs DHHS to report by November 1, 2019 on a more sustainable service model for group home residents, with reinvested State funds to support new funding models and equalize direct support personnel wages.

Excludes residents supported by the North Carolina Innovations Waiver from receiving funds under this act and protects residents from displacement if found ineligible for Medicaid services.
