Allows deponents lacking government-issued photographic identification to give testimony under penalty of perjury by signing a declaration instead of being sworn in.

Requires declarations to include case information, deponent's legal name and address, deposition date, a bolded perjury warning statement, deponent's signature with date, and signatures of two witnesses present at signing.

Provides that declarations have the same legal effect as affirmations under G.S. 14-209, with willful perjury subject to the same penalties.

Requires deponent declarations to be attached to the deposition transcript as an exhibit and filed with the transcript.

Effective October 1, 2019, applying to all depositions taken on or after that date.
