Requires dental services benefit plan insurers to use fair and accurate designation and rating systems for network dentists.

Mandates insurers disclose to consumers and dentists the basis for dental designations or ratings, including claims data, practice criteria, guidelines, or other methodologies used.

Establishes a mechanism allowing dentists to challenge and correct erroneous designations, data, or methodologies in rating or profiling systems.

Expands health benefit plan identification card requirements to include an indication of whether the plan is fully insured or self-funded, noting fully insured plans with "NCDOI" designation.

Applies insurance identification card requirements to the State Health Plan beginning October 1, 2021.
