Requires in-network health services facilities to provide insured patients with at least 72 hours' advanced written notification of any out-of-network health care providers who will be involved in their care, or notice on the day of appointment if less than 72 hours' notice is possible.

Applies to health care providers in anesthesia, emergency services, pathology, radiology, and those assisting physicians in these fields who may render services at in-network facilities.

Written notice must identify all out-of-network providers involved in the patient's care and include estimated costs for services rendered by those out-of-network providers.

For emergency services, facilities must provide written notice to the insured as soon as reasonably possible rather than the standard 72-hour requirement.

Becomes effective January 1, 2022, and applies to contracts entered into, amended, or renewed on or after that date; defers to federal Consolidated Appropriations Act, 2021 (P.L. 116-260) if any conflict exists.
