Enacts the North Carolina Medical Cannabis Act to allow qualified patients with a physician's written certification to legally possess and use cannabis for medical purposes without criminal penalties.

Establishes a regulated medical cannabis supply system administered by the North Carolina Department of Agriculture and Consumer Services, with licensed medical cannabis centers, producers of medical cannabis, and producers of cannabis-infused products.

Protects qualified patients, designated caregivers (age 21+), and physicians from arrest, prosecution, and penalties for medical cannabis use consistent with the Act, provided possession does not exceed an "adequate supply" as determined by a physician.

Requires licenses for all cannabis businesses with initial and renewal fees of $5,000, plus monthly fees equal to 10% of gross revenue from sales (15% for vertically integrated operations).

Prohibits smoking cannabis in public places, school grounds, correctional facilities, operating vehicles while impaired by cannabis, and selling to non-qualified individuals; establishes affirmative defenses for medical necessity claims.
