Authorizes the Commissioner of Insurance to establish and operate a state-run Health Benefits Exchange in compliance with the federal Patient Protection and Affordable Care Act (P.L. 111-148) and other applicable federal law.

Grants the Commissioner authority to change programs, adopt or amend rules, establish policies and guidelines, apply for federal funds, and create advisory boards necessary to implement and operate the Exchange.

Modifies G.S. 143B-24 to remove the previous prohibition on state actions toward forming a state-run Health Benefit Exchange while preserving the General Assembly's authority to define the state's level of interaction with federal exchanges.

Appropriates $100,000 in recurring General Fund moneys to the Department of Insurance for fiscal year 2024-2025 to establish and operate the state-run Health Benefits Exchange.

Becomes effective July 1, 2024, except as otherwise provided in the act.
