State Board of Elections must develop a sample voter registration form specifically for organized voter registration drives that is visually distinct from the official application and includes a disclaimer that it is not an official registration form

Sample forms must provide information on where to obtain and return official registration forms, how to register online, and important election dates, but cannot include spaces for individuals to fill in personal information

Organizations conducting voter registration drives must register with the county board of elections, providing organization name, structure, number of forms needed, whether participants are paid or volunteer, duration, and location of the drive

Using official voter registration application forms for organized voter registration drives is prohibited and constitutes a Class 2 misdemeanor

Effective upon becoming law and applies to voter registration drives conducted on or after that date
