Requires the NC Division of Health Benefits to submit a request to CMS within 90 days to extend Medicaid personal care services coverage to adult care home residents whose income exceeds State-County Special Assistance limits

Sets income eligibility thresholds at 180% of the federal poverty level for those who would qualify at the basic Special Assistance rate, and 200% of the federal poverty level for those who would qualify at the enhanced rate

Targets dual-eligible individuals (those on both Medicare and Medicaid) as an alternative to nursing home placement

Requires that any new Medicaid coverage costs must be offset by savings or cost avoidance

Implementation contingent on both CMS approval and meeting all specified goals including compliance with applicable legal requirements
