Large group health benefit plans in North Carolina must provide coverage for fertility diagnostic care, fertility treatment, and fertility preservation services (cryopreservation of gametes and embryos)

Coverage must include at least three in vitro fertilization (IVF) cycles per insured, including egg retrieval and fresh or frozen embryo transfer

Religious institutions and self-insured group health plans are exempt from the coverage requirements

Procedures must be performed at licensed healthcare facilities and conform to American Society of Reproductive Medicine guidelines to qualify for coverage

Experimental fertility procedures and nonmedical costs for donor gametes, donor embryos, or surrogacy are excluded from required coverage; effective October 1, 2025
