Appropriates $2,250,000 in nonrecurring funds for each year of the 2025-2027 fiscal biennium to establish a Fertility Preservation Pilot Program running from July 1, 2025 through June 30, 2032

Grants distributed equally to three medical institutions: Duke University School of Medicine's Onco-Fertility Program, UNC-Chapel Hill School of Medicine's Fertility Clinic, and Atrium Health Levine Cancer Center's REhope Program

Eligible participants include cancer patients whose fertility is jeopardized by diagnosis or treatment who are uninsured, lack comprehensive fertility coverage, or face out-of-pocket costs exceeding $1,000, with priority for those with household income below 300% of federal poverty level

Maximum financial assistance caps set at $12,000 for female participants (oocyte retrieval, cryopreservation, five years storage), $1,500 for male participants (sperm retrieval, cryopreservation, five years storage), and $26,000 per participant for IVF or IUI services

Grantees must submit annual reports beginning February 1, 2028 through February 1, 2033 detailing expenditures, services provided, participants served, and program effectiveness evaluations
