Establishes statutory definitions for "biological sex," "male," "female," "man," "woman," "boy," "girl," "mother," and "father" based on reproductive anatomy present at birth, applying these definitions throughout North Carolina General Statutes

Requires restrooms, changing facilities, and sleeping quarters in covered facilities (prisons, juvenile detention centers, domestic violence centers, rape crisis centers, and public schools receiving state funds) to be used by only one biological sex at a time

Creates a private cause of action allowing individuals to sue covered facilities that permit or fail to prevent persons of the opposite biological sex from using single-sex restrooms, changing facilities, or sleeping quarters, with potential remedies including actual damages, punitive damages, and attorney's fees

Eliminates the ability to change the sex designation on North Carolina birth certificates based on sex reassignment surgery, limiting new birth certificates to parentage-related changes only

Requires driver's licenses to display sex as reflected on the applicant's birth certificate rather than self-reported sex, effective upon enactment
