Appropriates $2,308,273 for FY 2025-2026 and $2,291,650 for FY 2026-2027 from the General Fund to the Division of Public Health for sickle cell disease programs and services

Allocates $1,316,364 annually to six comprehensive sickle cell medical centers (Atrium Health, Duke, ECU, Mission Hospitals, UNC-Chapel Hill, and Wake Forest) for patient support including social, behavioral health, and transition services

Requires each medical center to use $75,000 annually to create a full-time Transition Coordinator position to help patients move from pediatric to adult care

Provides $353,250 annually in grants to community-based nonprofit organizations serving sickle cell patients, prioritizing underserved counties

Funds NC Sickle Cell Syndrome Program operations including $362,036 annually for new staff positions and $260,000 annually to distribute evidence-based toolkits to emergency departments statewide
