Appropriates $7 million annually for the 2025-2027 biennium to establish a Home Modification Grant Program providing up to $10,000 per household for seniors age 65+ with income at or below 250% of federal poverty level for wheelchair ramps, bathroom safety improvements, doorway widening, and stairlifts

Appropriates $42.25 million annually for the 2025-2027 biennium for supportive in-home services, including expanded daily activity assistance, home health agency services, grants to Area Agencies on Aging and local departments, and a Caregiver Assistance Program offering up to $500 monthly to family members caring for eligible seniors

Appropriates $1.5 million in nonrecurring funds and $750,000 annually in recurring funds to develop a statewide hotline and online portal to help seniors navigate aging-in-place resources, with target operational date of December 1, 2026

Requires priority consideration for eligible seniors in rural and underserved communities and mandates use of licensed North Carolina contractors for home modifications

Requires annual reporting to the Joint Legislative Oversight Committee on Health and Human Services beginning April 1, 2027, detailing grants awarded, financial assistance provided, and specific modifications funded
