Creates a Senior Telehealth Assistance Program with $2 million annually to help seniors purchase telehealth equipment and internet access, capped at $1,000 per household, and expands existing telehealth infrastructure grants to prioritize practices serving elderly patients

Allocates $33 million annually to expand non-Medicaid home-based healthcare services and establish a Caregiver Assistance Program providing up to $500 monthly stipends to family members caring for seniors with household incomes at or below 250% of the federal poverty level

Establishes a $7 million annual Senior Home Safety Program offering grants and below-market loans for home modifications such as wheelchair ramps, grab bars, stairlifts, and doorway widening, with priority for disabled seniors or those below 250% of the federal poverty level
