Prohibits 340B covered entities (certain hospitals) from charging patients more than the acquisition cost for drugs purchased at discounted 340B Program prices, with violations constituting unfair or deceptive trade practices under G.S. 75-1.1

Requires covered entities to submit annual reports to the Department by October 1, detailing 340B drug acquisition costs, payments received, and prescription volumes broken down by payor type (Medicaid, Medicare, commercial insurance, uninsured)

Mandates disclosure of whether entities passed 340B discounts to patients through lower cost-sharing or provided sliding fee scales for patients with family income below 200% of the federal poverty level

Requires reporting of payments made to contract pharmacies, pharmacy benefits managers, and other third parties for 340B program-related services, including the number and location of contract pharmacies (in-state vs. out-of-state)

Department must publish all covered entity reports on its public website; first transparency reports due October 1, 2026
