Health care providers must furnish free copies of patient medical records when requested by the patient or authorized representative if records are for transferring care to another provider.

For other requests with patient authorization, providers may charge no more than $20 for the first 25 pages and $0.75 per page thereafter in paper or facsimile format, or $30 for the first 25 pages and $0.25 per page thereafter in electronic or digital format.

Charges include all administration fees, retrieval fees, and postage expenses and apply to both general medical records requests and no-fault insurance claims.

Medical institutions must promptly furnish written reports and permit inspection and copying of records regarding treatment history, condition, dates, and costs when requested by a basic no-fault insurer investigating a claim.

The same fee schedule applies when medical providers furnish records or bills to basic no-fault insurers, with identical rates for paper and electronic formats.
