Amends immunization data reporting requirements to allow the state department of health to establish an immunization information system and require health care providers to report childhood immunizations within four weeks of vaccination using electronic media.

Providers who fail to submit immunization reports within four weeks may not order or receive vaccines from the North Dakota immunization program and the department shall report the violation to the provider's occupational licensing entity.

Allows health care providers, schools, early childhood facilities, postsecondary institutions, and health units to exchange immunization data including the date and type of immunization administered.

Establishes a provider choice system within the North Dakota immunization program allowing participating health care providers to select any licensed vaccine, including combination vaccines, recommended by the federal advisory committee on immunization practices, except during disasters or public health emergencies.

Appropriates $1,500,000 from the general fund to the state department of health for the biennium beginning July 1, 2011, through June 30, 2013, to fund vaccine purchases through the federal vaccine purchasing contract for distribution to public health units.
