Establishes three separate two-year pilot programs (July 1, 2013 - June 30, 2015) for independent case managers within counties located entirely on Indian reservations: one under the Service Payments for Elderly and Disabled program, one under Medical Assistance Home and Community-Based Services, and one under Expanded Service Payments for Elderly and Disabled.

Qualifies independent case managers as licensed social workers who either meet private practice requirements or work for non-county agencies, and who enroll as qualified service providers under state rules.

Authorizes case managers to determine and redetermine eligibility for benefits, provide case management services to recipients, and conduct functional assessments of applicants and recipients, all under state direction and supervision.

Expires automatically on June 30, 2015.
