Defines "personal identifying information" to include name, address, telephone number, driver's license number, social security number, employer information, employee identification number, mother's maiden name, financial account numbers, and birth/death/marriage certificates.

Establishes penalties for using another person's identifying information to obtain credit, money, goods, or services without authorization: Class B felony if value exceeds $1,000, Class C felony otherwise, and Class A felony for second or subsequent offenses.

Creates offense for using identifying information to interfere with contracts or service agreements: Class A misdemeanor for first offense, Class C felony for second or subsequent offense, regardless of economic loss to the victim.

Requires prosecution to be commenced within six years after the victim discovers the offense.

Allows consolidation of multiple violations committed in different counties for prosecution in any county where one offense occurred.
