Amends income calculation methods for self-employed applicants by allowing use of either the previous one year of adjusted gross income or the average of the previous three years, with allowances for recently established or terminated businesses to use income statements or other reliable information.

Modifies the income eligibility limit from "net gross income" to "net income eligibility equivalent" of 160 percent of the poverty line for the children's health insurance program.

Requires the Department of Human Services to prepare, submit, and implement a plan for eligibility determinations aligned with modified adjusted gross income methodologies as required by 42 U.S.C. 1396a(e)(14).

Establishes plan requirements including private insurance contracts, conversion options, actuarial equivalence to benchmark plans, and coverage for inpatient/outpatient services, psychiatric services, prescriptions, preventive care, dental/vision services, and prenatal services.

Becomes effective upon implementation of the Department of Human Services' new eligibility system or upon completion of system changes required for compliance with the Patient Protection and Affordable Care Act.
