One or both parties to a marriage may elect to change their middle name by entering the new middle name on the marriage license application.

Acceptable middle name options consist of: the premarriage surname or former surname; the premarriage middle name and premarriage surname or former surname; or a hyphenated combination of the premarriage middle name and premarriage surname or former surname.

Compliance with these middle name provisions satisfies the satisfactory evidence requirements of section 39-06-07.1.
