Director of Transportation may issue special identifying certificates to mobility-impaired applicants for a fee of $3 per year or part of a year upon submission of completed application and statement from qualified physician, physician assistant, chiropractor, or advanced practice registered nurse.

Director may waive the requirement for a medical statement if the applicant previously submitted certification that their impairment is not reversible.

Certificates are valid for up to three years as determined by the director and must measure 9.5 inches in height by 3 inches in width, displaying the international symbol of access in white on blue.

Temporary certificates valid for up to three months may be issued for $3, with a maximum of one additional temporary certificate available, and extensions of up to three additional months permitted with supporting medical documentation.

Healthcare providers who provide false statements certifying someone as mobility impaired are guilty of an infraction with a minimum fine of $100.
