Requires poll clerks to verify identification including residential address and date of birth before delivering ballots, accepting driver's licenses, tribal government IDs, state-issued identification, or alternative forms prescribed by the secretary of state.

Poll clerks must request, correct, and update any incorrect or incomplete voter information from the central voter file when verifying eligibility or entering names into the pollbook.

Individuals lacking approved identification must be offered the opportunity to complete a provisional ballot application containing their full legal name, date of birth, and residential address, with the ballot clearly marked as "provisional."

County auditor must mail a postcard the day after the election to each provisional ballot voter requiring them to certify under oath that they resided in the precinct for 30 days before the election and were qualified to vote, with confirmation required before the canvassing board meets for the ballot to be counted.

County auditor must report any individual found to have fraudulently cast a provisional ballot or fraudulently certified their eligibility to the state's attorney for prosecution under section 16.1-01-12.
