Creates new section 65-04-04.4 establishing employer responsibility for the first $250 of medical expenses related to compensable workplace injuries, or the first $350 if a claim is filed more than 14 days after the employer received notice of injury.

Authorizes the Workforce Safety and Insurance organization to impose penalties up to 125% of unpaid medical expense reimbursements if employers fail to pay within 30 days, collectible through civil action.

Amends premium payment default procedures to allow the organization to declare an employer uninsured after 45 days of non-payment and to decline coverage to uninsured or delinquent employers.

Increases penalties for willful misrepresentation of payroll from $2,000 to $12,000 plus three times the premium difference, and raises the felony threshold for willful non-compliance from $500,000 to $1,000,000 in premium due.

Repeals section 65-05-07.2 and consolidates medical expense assessment provisions into the new section 65-04-04.4, effective for injuries occurring after July 31, 1995.
