Creates chapter 23-49 of the North Dakota Century Code establishing hospital discharge planning requirements for all patients.

Requires hospitals to adopt written discharge planning processes and identify patients likely to suffer adverse health consequences without adequate discharge planning.

Mandates hospitals document discharge plans in medical records and communicate plans to patients, informal caregivers, or legal representatives as appropriate.

Requires hospitals to educate and train patients, informal caregivers, or representatives on posthospital care including medications, treatment modalities, therapies, and appointments tailored to identified needs.

Allows education and training to include repeated review of materials with patients, caregivers, or representatives to ensure understanding of discharge care instructions.
