Creates new section 54-52.1-04.19 requiring public employee health insurance boards to provide coverage for infertility diagnosis, fertility treatment, and standard fertility preservation services if recommended and medically necessary.

Coverage must include at least three completed cycles of intrauterine insemination, fertility treatments including in vitro fertilization and embryo transfers, third-party reproductive care (donated eggs, sperm, embryos, or gestational carriers), and fertility preservation services for individuals with medical conditions affecting fertility.

Prohibits limiting benefits based on copayments, deductibles, coinsurance, benefit maximums, waiting periods, exclusions on fertility medications, discriminatory treatment of protected classes, or pre-existing condition exclusions, with a maximum benefit limit of at least $50,000 per covered individual.

Applies to health coverage beginning after June 30, 2021 through June 30, 2023, requiring the Public Employees Retirement System to submit a bill to the 68th legislature by July 31, 2023 with a report on utilization, costs, and recommendations for extending coverage.

Amends section 26.1-36.6-03 to apply the new fertility health benefit requirements to self-insurance health plans under the same standards and guidelines as group accident and health insurance policies.
