Designated caregiver applicants must submit a $50 nonrefundable annual application fee, a completed registry identification card form, an authorization form signed by the qualifying patient, a 2x2 inch photograph, and other department-required materials.

A registered qualifying patient may have no more than five registered designated caregivers.

The medical marijuana advisory board shall consist of six members appointed by the governor (a health care provider, department representative, manufacturing facility representative, dispensary representative, registered qualifying patient, and licensed pharmacist) plus two legislative members appointed by the chairman of legislative management.

The advisory board shall advise the department on medical marijuana program implementation, receive status reports, and provide recommendations to the department and legislative management.

The state health officer serves as an ex officio voting member and chairman of the advisory board, with board member terms lasting two years and eligible for reappointment.
