Prohibits prior authorization requirements for six medication classes for individuals age 21 and older: antipsychotics, antidepressants, anticonvulsants, antiretrovirals for HIV, antineoplastic agents for cancer, and stimulant medications for ADHD/ADD, except for prescribers exceeding two times the top ten prescribers' rate.

Prohibits prior authorization for the same medication classes for individuals under 21, except stimulant medications for ADHD do not include the ADD exception, and allows prior authorization when individuals have five or more concurrent psychotropic medication prescriptions.

Requires prior authorization for individuals under 21 with five or more concurrent prescriptions for antipsychotics, antidepressants, anticonvulsants, benzodiazepines, mood stabilizers, sedatives, hypnotics, or ADHD medications, but requires department approval after prescriber consultation with a board-certified pediatric psychiatrist.

Permits cost-based exceptions to allow prior authorization when brand name drugs cost less than generics or vice versa, and when medications are classified as line extension drugs (new formulations but excluding abuse-deterrent formulations).

Exempts these restrictions when prior authorization is required by Centers for Medicare and Medicaid Services.
