Health carriers must provide coverage for licensed pharmacists to deliver comprehensive medication management services to enrollees who elect to participate in such programs.

Enrollees are eligible for these services if they take five or more chronic medications, were hospitalized for specific conditions (heart failure, pneumonia, myocardial infarction, mood disorder, or COPD), or have comorbid diabetes with hypertension or hyperlipidemia.

Health carriers must maintain adequate networks of participating pharmacists, accept credentialing for pharmacists employed by contracted health care facilities, and maintain electronically searchable directories of eligible pharmacists with annual accuracy audits.

Services may be delivered via telehealth and include medication assessments, treatment plans, therapy monitoring, comprehensive medication reviews, patient counseling, drug therapy initiation or modification, medication administration, and laboratory test ordering and interpretation.

An advisory committee must develop best practice recommendations by June 30, 2024, and requirements apply to all health benefit plans issued or renewed after December 31, 2024.
