Creates the Family Home Visitation Act establishing voluntary home visitation programs for families with children five years or younger or pregnant persons, delivered by nurses, social workers, and trained health professionals to improve maternal/child health outcomes, reduce preterm births, and increase school readiness

Requires the Department of Health and Human Services to submit a Medicaid state plan amendment by October 1, 2025 to implement targeted case management for evidence-based nurse home visiting services for postpartum mothers and children six months or younger enrolled in Medicaid

Mandates state-funded programs meet evidence-based standards including certification by a national organization or institution, comprehensive quality standards, trained staff, and fidelity to the program model

Requires annual electronic reports to the Legislature from February 15, 2026 through February 15, 2028 detailing program types, locations, families served, outcomes, expenditures, and demographic data

Funds nurse home visiting services through the Medicaid Managed Care Excess Profit Fund rather than the General Fund, adding these services to the fund's authorized uses
