Coverage is required when testing is supported by FDA-approved indications, Medicare/Medicaid coverage determinations, or nationally recognized clinical practice guidelines

Prior authorization decisions must be made within 72 hours for non-urgent requests and 24 hours for urgent requests, with patients and providers having access to a clear appeals process

Biomarker testing includes single-analyte tests, multi-plex panels, protein expression analysis, and whole exome, genome, or transcriptome sequencing to measure gene mutations or other biological indicators
