Coverage may be limited to the most appropriate device deemed medically necessary by the treating physician, including repair or replacement when determined appropriate

Prohibits insurers from imposing annual or lifetime dollar maximums specific to prosthetics and orthotics coverage, though general policy maximums and standard deductibles/copayments still apply

Exempts short-term major medical policies of six months or less and limited-benefit or specified-disease policies from the coverage requirement

Defines prosthetics as artificial legs and arms with associated components, and orthotics as custom-fitted or custom-fabricated devices applied to correct deformities, improve function, or relieve disease symptoms
