School districts must notify parents, guardians, and educational decisionmakers at least 15 days before administering surveys requesting sexual, mental health, medical, health-risk behavior, religious, or political information from students, or nonanonymous surveys about drug, vape, alcohol, or tobacco use

Parents have the right to request a copy of the survey, review it in person at the school, and exempt their child from participating

Surveys requesting sexual information from students are prohibited for kindergarten through grade six

School personnel cannot disclose personally identifiable student information from surveys unless required by federal or state law

Required notifications must be sent through the school's electronic notification system or physical mail and must describe the survey's nature, purpose, age-appropriateness, data use, access, privacy protections, and disclosure plans
