Adopts the Uniform Health-Care Decisions Act (Sections 1-30) to establish comprehensive rules for advance health-care directives, including powers of attorney for health care and health-care instructions

Allows individuals to appoint agents to make health-care decisions on their behalf when they lack capacity, and establishes a priority list for default surrogates (spouse, adult child, parent, cohabitant, sibling, grandchild/grandparent) when no agent is designated

Creates specific provisions for advance mental health-care directives, allowing individuals to direct that certain portions cannot be revoked during specified psychiatric or psychological events

Establishes capacity determination procedures, including a presumption of capacity that can be rebutted by physician examination, with rights to object and petition the court within 7 days

Provides statutory damages of $25,000-$50,000 for violations including falsifying directives, concealing documents, or health-care institutions intentionally failing to comply with valid directives
